The Agent has a contract with the insurance companies in order to represent a specific insurance company. A broker is person who also contracts with <strong>insurance </strong>company but officially acts on behalf of you the client. All agent and agencies are compensated by the insurance companies and therefore are at no cost to you. The difference between agent and agency is that agent is individual contracted with insurance companies and agency usually has many agent working under umbrella of the agency. In most cases agents working under agency are employees.<br />
Agents role is to find best possible and the most affordable option for your money. In most cases there is no incentive on the agent’s part of which company you decide to go with. How agents get paid by the insurance company is strictly regulated by the insurance commissioner. When looking for agency or agent make sure that they are brokers who represent as many health insurance companies as possible. There are dozens of plans on the market, therefore finding agent who can get you the best possible plan for your money will save you thousands long term. Just looking a rate quote and benefit summary is only half of the story when choosing health insurance. Some plans exclude certain benefits or apply caps to other types of coverage. Agents know the lay of the land and you would personally not want to purchase health insurance just based on a rate quote.<br />
Some insurance companies have captive agent representing them. Captive agents can only represent one insurance company. They are essentially employees for that company. I would avoid such companies as they may not have the best interest of the client at heart. If a company cannot quote multiple carriers, beware. The major carriers in California are Blue Cross of California, Blue Shield of California, Health Net, Pacificare, and Kaiser. Beyond these five, we do not feel very secure about the others.<br />

The most important concept of determining your monthly <strong>health insurance</strong> premium is similar to with all the insurance company. The differences will depend and how efficiently insurance company is ran. Therefore if you think are just getting great deal from one insurance company you are probably giving up some benefits that you are not aware of. It could be exclusion, limitation on how much plan pays for something, deductible, co-insurance and many more very creative ways that insurance companies taking out benefits to make it more affordable. It is nearly impossible to compare plan from one company to a plan from another. <strong>Insurance</strong> companies have to compete and almost never have plan designs as the competing insurance companies in your state. For example in California Blue Cross and Blue Shield are two separate insurance companies that are competing against each other. Both Blue Cross of California and Blue Shield of California what might look like have same or similar plans. That what you might think and make your purchase decision on price alone. Most people when shopping for health insurance are only looking at deductibles and co-pays. What is going to make a big monthly price of health insurance is the plan design it self.<br />

In some cases you can have exact some plan except one would cover doctor office visits and one would not and you will same your self up $100 per month. If my math adds up, if you take a plan that does not cover doctor office visits and you will go to a doctor once per month well that&#8217;s $100(your monthly savings) minus $65(cost of a doctor office visit if you pay out of your own pocket. There you have it is a simple math. The cost of actual catastrophic health insurance is actually really cheap. It is all of those add-ons that you have with a plan that make it expensive.<br />
Think about this if you have car insurance you know that it will only cover you if you get in a car accident. Car insurance does not pay for any &#8220;preventative&#8221; care. Car insurance does not pay for your gas, oil changes, upgrades to your car and &#8220;check-ups&#8221;. The reality is health insurance is actually inexpensive in the Untied States if you get health insurance for what it was designed for, major events. The problem with health insurance is not health insurance it is all the people that are leaving large corporate plans that did pay for everything. Well when you work for a company they either pay 100% for your health insurance or a large portion of it. Then the remainder that you have to pay they take it out of your check once every two weeks, this way it does not look that much. If you get used to the way your large company group plan worked, that the company played for, and you are out there looking for the similar plan on your own, of course it is going to be expensive.<br />

Some insurance companies have co-pay for emergency room visits some do not. Some insurance companies have called &#8220;daily facility fees&#8221; on to of your deductible. If you were looking for car insurance that covered your gas, oil changes, basic maintenance and checkups, the cost of that coverage would be astronomical. When looking at the plans all you should be concerned with is the worst case scenario that I can be out of pocket with this plan.<br />

Employers are facing tremendous pressure today to handle the increasing costs of health care insurance. The <strong>health care insurance </strong>industry is as volatile as has been seen in more than a decade. Many employers are wondering how they can afford to maintain their benefits.<br />
This article discusses some of the many elements contributing to these escalating costs and provides some basic strategies for employers to explore to improve the cost and benefit offering.<br />
Health insurers are essentially repackagers of the vast American health care system. Most employer policies include physician services, hospitalization, mental health and prescription benefits.<br />
The health insurers consolidate our disparate system to give the consumer, through the employer, a conduit to health insurance. Herein lies a part of the challenge each insurer faces. Each part of the system is seeing its&#8217; component rise in pricing.<br />
Prescription costs lead the way with an average trend factor of around 28%. Pharmacy costs have consistently risen in the 15 to 20% range each year for the last three years says Jeff Smith of Milliman&#8217;s Columbus, Ohio office.<br />
Every carrier is faced with the challenge of changing consumer-purchasing patterns. Pick up any magazine and see the many advertisements that attempt to compel the consumer to ask their physician for a prescription by name. The patient and not the physician now often facilitate the prescription purchase. This changing purchasing pattern is dramatically altering pricing. The average percentage increase for small employer health insurance is between 14-15%. The prescription component is heavily weighted in this amount.<br />
The Health Insurance Portability and Accountability Act (HIPAA) or the Katzenbaum Kennedy Bill passed in 1996 has impacted how health insurers handle much of their business. Group health products must be guaranteed issue to any small employer applicant who adequately meets contribution and participation guidelines.<br />
But paramount for any group is the overall health status of the group. If your group has a combination of health conditions that will constitute a marginal risk, you may be rated-up prohibitively by the various <strong>insurance</strong> vendors. Work with an insurance agent to help facilitate this process so that all of your options are evaluated.<br />
The following are among the many other reasons that costs for health insurance are increasing so dramatically:<br />
-Technological advances in the health care industry.<br />
-An aging U.S. Population.<br />
-Physicians and hospitals are banning together to regain negotiating clout with insurance vendors.<br />
-Legislation has created far more regulatory requirements.<br />
-Overutilization of services by Americans.<br />
As described in the opening paragraph, health insurers as private labelers of our health care system must identify strategies to help relieve the pressures of these various issues. Each of these items is impacting profitability and an unprofitable industry is not ideal for purchasers, consumers or vendors and also creates much of the volatility each employer is facing on their renewal.<br />
Employers must begin to engage their employees in these cost increases. Employers can do the following things to increase awareness:<br />
-Provide a detailed breakdown of the overall cost for insurance and then show the employee contribution on payroll information.<br />
-Provide a payroll stuffer that demonstrates costs sharing or total employer cost.<br />
-During annual review periods ensure the employee understands the costs of employee benefits and note that the employer portion represents effectively additional income.<br />
-Increase the employee contribution while explaining the price increase.<br />
Employers must analyze their costs regularly. Each year the company will renew with their insurance vendor. The employer has the opportunity two to three months prior to evaluate the marketplace. There are many factors that will determine if the company is able to improve price, product or network.<br />
So what can an employer do to keep costs affordable? There are several steps that should be considered in attempting to keep costs affordable:<br />
-Evaluate your plan each year and ensure pricing is competitive.<br />
-Make annual plan changes to offset increases in pricing such as increasing copays or deductibles.<br />
-Consider changing your drug card to engage your employees more in the cost.<br />
-Evaluate employee contribution levels and consider changes in the percentage of overall costs that you provide.<br />
-Discuss the affordability of a health care offering with your employees and leadership to ensure each knows the pressure of maintaining such an offering.<br />
For many employers, the cost of offering health care is typically the in the top five highest costs behind costs such as payroll and raw materials. This high ranking in cost is growing exponentially and deserves the time to be carefully evaluated and assessed. As the consumer becomes more engaged in the complex purchasing decision for the employer, there may be a gradual enlightenment by some employees who will realize that everything has a price.<br />
Take the time to have your benefits evaluated by a seasoned professional who will carefully analyze your company&#8217;s benefit/price/risk equation for your health care offering so that you can optimize your position.<br />

			<content:encoded><![CDATA[<p>The results of an April 2004 Commonwealth Fund white paper* show that the economics of small business group insurance makes offering health benefits to employees is risky. The current lack of health insurance for individuals in Texas, as well as the rest of the country, is closely associated with the inadequacies of the small employer market. Of the approximately 44 million individual Americans without<strong> health insurance</strong>, over 80 percent come from working families. Nearly 50 percent of uninsured workers are either self-employed or work for firms with fewer than 25 employees.<br />
Small and Large Companies Benefits and Premiums<br />
Surveys of employers from 1989 to 2003 reveal that more rapid premium increases are forcing small firms to impose higher cost sharing. In 2003, premiums for small firms (3,199 workers) increased 15.5 percent, outpacing the 13.2 percent increase for large firms (200+ workers). From 2000 to 2003, deductibles among small firms increased 100 percent in PPO plans when employees use in-network providers and 131 percent when they use out-of-network providers. For large firms, deductibles in PPO plans increased 33 percent and 44 percent, respectively. Also in 2003, 40.3 percent of employees in the smallest firms contributed 41 percent or more of the total family premium, compared with only 11.2 percent of employees in large firms.<br />
Share of Premium Contribution<br />
In addition to paying higher deductibles, employees in small firms contribute a greater share of the premiums. In 2003, 40.3 percent of employees in the smallest firms contributed 41 percent or more of the total compared with only 11.2 percent of employees in large firms. Among all small firms, 38.2 percent of employees contributed 41 percent or more of the family premium. For single coverage, 7.6 percent of employees in the smallest firms contributed 41 percent or more of the premium, compared with 3 percent of employees in the largest firms. However, employees of the smallest companies were more likely to contribute none of the premium (61.6% vs. 14.0%).<br />
This increased cost sharing, especially of family plans, in small firms is consistent with the finding that small employers get less value for their premium dollar than large employers.<br />
Small Equals Less, Plus More Risk<br />
Small employers not only get less value than large employers when they provide health benefits, but they face greater financial risk in doing so. Lower value is a natural consequence of small size and the failure to join together in pooled purchasing groups with a long-term commitment to shared risk.<br />
In any given year, premium increases, the cost of single coverage, and employee contributions vary more from firm to firm for small than large firms. Small firms lack purchasing power in the insurance market and unlike larger companies, are unable to reduce insurance costs by bearing the risk themselves and self-insuring.<br />
This means a fundamental change in the small employer market is necessary. This change requires new options for helping small firms gain access to the advantages larger firms have in purchasing health benefits. Burdened with inherently higher administrative costs, having fewer lives over which to spread the risk of catastrophic costs, and lacking the purchasing power of large firms to negotiate with insurers, small employers are doomed under current practices to separate but unequal status.<br />
What nobody knows is how many individuals decide not to start a business because of the greater risk in the small employer market when purchasing health insurance. Yet if small employers are the principal source of innovation, as well as economic and job growth in the American economy, then this greater risk costs, not only small employers and their workers, but the overall American economy.<br />
